Filing Claims
Your claims for benefits under this plan will automatically be filed by Medicare with United Healthcare, the insurance company, if you enroll in the Medicare Cross-Over Plan. However, for expenses (such as ambulance service) which are not considered eligible under Medicare, you must file a claim to receive benefits.

Claims for prescription drug expenses should be filed under the prescription drug program. See the Prescription Drug section for further information.
