Health Insurance Portability and Accountability Act (HIPAA) |

This plan operates in accordance with regulations under the Health Insurance Portability and Accountability Act ("HIPAA") with respect to protected health information ("PHI"). For purposes of the plan, PHI generally consists of individually identifiable information about you or your dependents, including health and demographic information, that relates to your or their eligibility for all group health benefits under the plan. Additional information about your rights under HIPAA are provided separately in a Notice of Privacy Practices.
